ADOPTION CONTRACT FORM
This form is required to continue and finalize the process of border collie adoption.
If you have not already submitted the on-line border collie adoption form, you should
print and fill out the printable version of the form and mail it along with this contract
agreement to the address indicated below.
Please read over this contract carefully. It explains the requirements for adopting a
border collie and your commitment for care and handling of the animal. If you have any
questions regarding this agreement please contact us at the email address below.
A signed version of the Border Collie Adoption Agreement must be mailed
to the address below:
As a condition for adopting a border collie, I hereby agree to the following:
___I agree to have the animal spayed or neutered within two months of adoption.
(Note that in most cases, this will have already been accomplished.)
___I agree to maintain the animal up-to-date on all required shots, including parvo
and rabies shots, and heartworm medicine.
___I agree to allow a representative of a Border Collie Rescue organization to visit my
home for the purpose of evaluating the environment under which the border collie would live.
I understand that border collies have different needs and not all environments are suitable
for all border collies.
___I agree to provide suitable shelter and environment for the border collie, including,
but not limited to providing a dog house, dog run, adequate fencing to ensure that the dog
is not allowed to run the streets, and protection from extreme cold or heat.
___I understand that failure to adhere to the above requirements may be grounds for
immediate retrieval of the animal by a Border Collie Rescue representative.
